Have you had recent imaging? If you had a recent MRI, CT, or X-ray, let us know where and when it was done. Bring the report or images if you have them; our team can also assist with obtaining relevant records and imaging when needed.
Bring your list. Current medications, prior treatments and injections, and what helped or did not.
Bring a question. The visit is a conversation. Write down what you want to be able to do again.

- What are the office hours?
- Flowood: Monday to Friday, 8 AM to 5 PM. Brookhaven: Monday to Thursday, 8 AM to 5 PM.
- Does Dr. Smitherman work anywhere else?
- Dr. Smitherman also performs procedures at S.E. Lackey Memorial Hospital in Forest and in Batesville. Clinic appointments are at the Flowood and Brookhaven offices.
- What should I bring to my first visit?

- How do referring providers send a referral?
- By fax to (601) 348-8716, by email to referrals@sipsmd.com, or by calling the referral team at (601) 891-5524. A standard referral out of your EHR is enough.
