Southern Interventional Pain Specialists, PLLC (SIPS) complies with applicable federal civil-rights laws and does not discriminate on the basis of race, color, national origin, sex, age, or disability in its health programs or activities.
SIPS provides free language assistance to individuals with limited English proficiency and, when needed for effective communication, appropriate auxiliary aids, accessible formats, and reasonable modifications for individuals with disabilities.

Civil rights questions or grievances
If you believe SIPS failed to provide these services or discriminated in another way, you may contact or submit a grievance to the SIPS Civil Rights Contact by mail, phone, fax, or email. Assistance with filing a grievance is available upon request.

SIPS will not retaliate against a person for requesting assistance, asserting civil rights, filing a grievance or complaint, or participating in an investigation.
