Website privacy

The appointment request form on this site asks for your name, date of birth, phone number, and email address, and for what you choose to tell us about your request: where it hurts, how long it has hurt, who referred you, and your insurance plan. It is used only to contact you, schedule your visit, or answer your question.

When you send the form, it goes straight from your browser to a secure system that Synaptix Lab, the practice's website developer, runs for the practice under a business associate agreement. That agreement requires Synaptix to protect your information the way HIPAA requires. Your request is not stored on this website's server and is never sent to analytics or advertising companies. The office gets a copy of your request by email, inside the practice's own email system. From there, your information is handled by the practice under its Notice of Privacy Practices.

You can always call or email the office instead. If you email, please keep records and images for your visit.

Referrals from other providers, which include patient identity, never go through this website. They are sent by fax, email, or phone directly to the office.

The site sets no cookies and sends no visit data to advertising companies, Google, or Meta. It counts visits with Umami, an analytics tool that the practice's website developer, Synaptix Lab, runs on its own server for the practice. It records the full address of each page viewed, including any campaign tag in it, the site that sent you, your browser, device, screen size, and language, and the country, region, and city of the visit. It also records how fast each page loads, how far down a page you scroll, and which phone numbers, email addresses, directions, documents, and outside links you tap. It does not store your IP address or your name, and it records nothing you type. The site records the same information whether or not your browser sends a Do Not Track signal.

Fonts are loaded from Google Fonts and Fontshare, which receive your browser's request for the font files and the address of this website, nothing else. The Directions buttons on the Locations page open Google Maps, which is Google's service under Google's privacy policy.
